I would like to take this opportunity to thank the nursing staff and rehab program at CHHC. During my stay, the nursing staff was wonderful on all levels. I would particularly like to thank the rehab staff and single out David and Linda. David’s patient persistence and encouragement really motivated me to take every opportunity to speed my recovery. Linda’s common sense approach to handling my personal needs both before and after leaving CHHC, I still remember and use every day. I know my present state of mind as I continue my rehab I can directly link to David and Linda’s insight and expertise.

It is with great appreciation that I express thanks to you, on behalf of myself and my husband for the extraordinary hospitality which you extended to us and to the Navy League of the United States at the ceremony on Thursday where he was awarded the honor of a place in the Navy League Hall of Fame. I thank you and all of the Cobble Hill Health Center staff for the way in which you made this a truly memorable and joyous event.
Thank you for all that you and your entire staff do every day to support and give thoughtful care to all the Cobble Hill Health Center residents. I am extremely appreciative of all that you have done to support me in getting the help needed to hopefully restore my husband to a level of functioning at which it will be possible to bring him home.

Dear Mr. Lewis:
I want to personally thank you on behalf of our residents and their families for all your help during our evacuation. Many obstacles were overcome because of your assistance. We had to move 195 residents in the period of one day and this would not have been accomplished without everyone working together.
Please thank all your staff on our behalf and let them know that this was truly a perfect example of the health care community showing ts caring and compassion for all those entrusted to us.

Good afternoon and welcome to my mother’s 100th birthday party. I would like to welcome all of you to such a momentous occasion. I am very thankful to all of the staff for all the attention that you’ve given to my mother. It’s thanks to you that we are reaping the blessings of this day.
I would also like to thank Yani, mother’s night nurse; she is truly attentive and caring. When I am held hostage by my mother, Yani will take a few minutes and sit with my mother and have a conversation with her, and this gives me a chance to get away and go home, (smile).
I would also like to give a special thanks to Doris for the beautiful cake she presented to my mother upstairs in the day room for her birthday celebration.
Also a special thanks to Renee, for the decorations and preparations for Mom’s birthday, also a special thanks to Irena for making Mom’s birthday special. And first of all I want to thank the Director Administrator, Ms. Syers, for introducing all of the staff members who contributed to Mom’s birthday party. All of you have really pulled together to make this milestone in Mother’s life very special.

My husband, William J., is a resident at Cobble Hill Health Center. William was admitted in January 2011. Since then I’ve gotten to know the staff on floor 3A and have observed their interaction with the residents and I commend them for their hard work.
Generally you have a good staff but there are those who go the extra mile. My husband is doing well and that is largely due to the care that is being provided by this facility. My thanks to all of you.

From Dr. Dhillon, to the Nurse Supervisor Ms. Concepcion and her staff, to her physical therapist Michelle and her occupational therapist. Most importantly was her social worker Arthurina Mathis. Ms. Mathis was not only responsive to my inquiries but proved to be a wealth of information. She made what could have been a very difficult situation more palpable……“

Everyone associated with my mothers’ care on “2 South” treated her with grace and kindness. From Dr. Dhillon, to the Nurse Supervisor Ms. Concepcion and her staff, to her physical therapist Michelle and her occupational therapist. Most importantly was her social worker Arthurina Mathis. Ms. Mathis was not only responsive to my inquiries but proved to be a wealth of information. She made what could have been a very difficult situation more palpable. The resource guide of Brooklyn Resources she gave me has been indispensable and I have passed on information to friends and colleagues who are also faced with the care of an aging relative. Ms. Mathis also contacted Revival Home Health Care who assessed my mother’s condition and were able to provide her with 12-hour daily home attendant services.
There are still challenges that I am faced with and may become acquainted with in the future, but the staff at Cobble Hill helped me to make a difficult transition and to do so with compassion and professionalism. Thank you for your leadership and I thank your staff for all that they did for my mother and all they continue to do on a daily basis.

On my first visit I had the pleasure of meeting her nurse Ms. Sandy who smiled often and would always have time to answer my questions. She was very professional and caring. I would often comment to Ms. Sandy how much better my mother looked since she was admitted to your facility.
My mother has expressive aphasia after two strokes but your staff would always take the time to figure out what she needed. I saw firsthand how hard the staff of 1B works and would still have smiles on their faces.
Ms. Cynthia Spaulding, my mother’s nurse assistant, took great care of her. Ms. Ceasar on the evening shift livened up the evenings with her upbeat personality and laughter. I think this is good for the morale of the residents. I would also like to thank Ms. Williams for making the discharge process relatively stress free. She took time in explaining all my mother’s treatments and medications. I would like to thank all the nursing staff that took care of her as she received superb care.
The whole admissions process started off with my speaking to Ms. Rifky over the phone. I had always taken care of my mother at home, but Ms. Rifky’s calm, reassuring voice and her patience in addressing all my concerns made me feel this was the best place for my mother’s recovery.
I would also like to thank the Physical Therapy staff for helping my mother. It is amazing how much physical and occupational therapy can do for residents to improve their level of functioning. Your staff is very friendly and accommodating. I met several people from housekeeping, accounting, dietary, security and the recreation departments who I noticed truly cared about the residents and their families.
Lastly, I would like to thank Ms. Christine Davis social worker who was instrumental in setting up my mother’s discharge and providing us with VNS services. Ms. Davis was very kind and caring,
Again I would like to give a heartfelt thanks to everyone who was involved in my mother’s care. I miss you already, Your institution is unique in its compassion for its residents.

I am writing to thank you for having chosen to assign Mr. Lesly Jacques to be my physical therapist during my stay at Cobble Hill Health Center, following my knee replacement surgery on my left knee, which was performed at Long Island College Hospital on December 2, 2009.
Mr. Jacques is an excellent physical therapist. He urged and encouraged me to do the exercises, which would help me regain flexibility and agility in my knee. He “set the bar” of difficulty high enough that I would, and have improved over time, even when I winced from the discomfort (no pain, no gain). He gave me time to relax between sets of exercises, and he complimented me on my efforts and accomplishments.
In addition, the occupational therapist assigned to me, Ms. Linda Manning, observed me in the rehabilitation center and in my room. She complimented me on my efforts to get around independently, and encouraged my efforts.
