The Bridge Between Hospital And Home: Subacute Care Unit
When the patient is ready to leave the
hospital but is not ready to resume independent living, Cobble Hill
Health Center’s subacute unit is the bridge to complete health.
Our registered nurses meet with potential subacute patients in the
hospital when a doctor or other health care provider refers them
to us. After the patient is admitted to our unit he or she will
be examined by a physician and evaluated by other members of the
team.
A comprehensive care
plan is designed to ensure a quick and complete recovery. The plan
will incorporate rehabilitation, psychological support and education
for the patient and family as well as the best nursing and medical
care. Our plan is reviewed and updated on a regular basis.
Our goal is to restore
our patients’ independence and help them return to community
living. Access to the latest medical interventions and a skilled
nursing staff, provide the ideal setting for swift recovery.

Stroke or heart attack requiring rehabilitation
Hip fracture

Surgical patients, i.e. colostomy, hip or knee replacement,
amputation or multiple fractures

Social Work Counseling
When the patient is ready to go home, our social work staff coordinates
discharge plans, offers support and counseling, arranges for adaptive
equipment and trains patients in their use at home. A smooth transition
from the subacute unit to the patient’s own home is our objective.
Our social work team will assist in arranging for home evaluation,
home care and other community services as needed.
