Facilitating Admissions
The courteous and streamlined admission process at Cobble Hill Health
Center guarantees that within 24 hours of referral, patients can
be resting comfortably in their room. With a full-time nurse on
duty for evaluation and an experienced financial coordinator familiar
with Medicaid, Medicare and many large HMOs, a patient can
be admitted to Cobble Hills Rehabilitation Institute quickly
and efficiently, even on weekends and evenings. Medicaid applications
can be processed and on-site patient assessments can be arranged.
We
know what a stressful time it is for patients and
caregivers when they must make a choice about Rehabilitation
Therapy.

Going Home
Our interdisciplinary team recommendations and in-facility assessments
by home-care nurses prior to discharge, ensure safe and timely discharges
to home care and the community. Adaptive devices, such as walkers,
commodes, wheelchairs and canes are provided. Social workers develop
strong lines of communication with families and follow-up once patients
are released.
