About Cobble Hill Health Center
Cobble Hill Health Center has been providing a continuum of residential and community-based services for over 20 years. Services offered at Cobble Hill include: short-term rehab unit, cardiac rehab unit, sub-acute medical care unit, Avram Dialysis Center, Alzheimer’s Resource Center and and Adult Day Health Care Program. We strive to provide culturally competent services. Our bilingual (Spanish and English) staff are able to meet diverse needs.

What is the Long-Term Home Health Care Program at Cobble Hill?
The Cobble Hill LTHHCP is a unique program also known as “nursing home without walls.” It includes nursing care, rehabilitation therapy and other health services delivered in the comfort of home on a long-term basis. These services may include heart and blood pressure monitoring, wound care, medication administration, as well as help with speech and simple movements. The care your loved one receives is personalized and can be modified as his or her needs change.

What are the advantages of Long-Term Home Health Care?
Long-Term Home Health Care allows your loved one to remain in a comfortable, familiar place near family and friends. Chronically ill patients, who would otherwise require a skilled nursing facility, get to stay where they want—in their own home.

Will the caregiver come when it’s convenient for me and my loved one?
Care is there when needed. This includes scheduling visits when they are convenient for your loved one and your family. When care visits are scheduled, your loved one’s and your family’s needs are taken into account.

How do I know my loved one is receiving the best possible care?
Our Long-Term Home Health Care services are licensed by the New York State Department of Health. We have highly skilled professionals:

  • Nurses to monitor health and administer medications and treatment
  • Respiratory, physical, occupational and speech therapists to help regain any abilities lost or impaired as a result of injury or illness
  • Home care aides to assist with bathing, dressing, cooking, shopping and other personal needs
  • Social workers to help obtain benefits and provide counseling

Who is eligible for Long-Term Home Health Care?
People with serious, chronic health problems as well as individuals recovering from an illness, an injury, or a recent hospitalization. The eligible clients must be residents of Brooklyn. They must be under a physician’s care and in need of preventive, restorative or supportive services.

What do you do to ensure that my loved one is receiving the best possible care?
We advocate for patients to obtain support services such as rehabilitation, medical day care, nursing home placement, enriched housing, Medicaid/Medicare, personal care, laboratory services, 24-hour emergency coverage, medical transportation, medical equipment and supplies.

Who pays for Long-Term Home Health Care?
Services we provide are covered under Medicaid and participants must meet Medicaid’s eligibility requirements.
