What Is Our Adult Day Health
Program?
It’s an organized day program that provides comprehensive health
and personal care, socialization, recreation, supervision and psychological
services. Who can participate? The chronically ill, those with physical
or cognitive impairments, the frail, the isolated elderly, the disabled
and people with disabling conditions that do not require 24-hour
a day care.
Our day and evening programs serve adults with physical
disabilities or cognitive impairment who do not require 24-hour
institutional care yet are not capable of full-time, independent
living. Jacquelyn Hernandez Adult Day Health Program is a medical
model, multi-disciplinary program designed to maintain maximum levels
of health and psychological well-being while providing respite for
family members/caregivers. Located at Cobble Hill Health Center,
a skilled nursing and rehabilitativefacility, participants have
ready access to: rehabilitation therapists, medical professionals
and social workers. In fact, the Day Center is the bridge to services
that help those with medical problems continue to live at home.
It’s a long-term care program that helps adults remain in their
community, rather than in a nursing home.
What
Is The Cost?
Medicaid covers all costs. We also accept private pay and some private
insurance companies.
Who Can Benefit From Adult
Day Health Care?
Participants and their caregivers.
How?
By allowing the participant to leave home during the day to socialize
and participate in stimulating activities. By giving the caregiver
a respite.
How Much Time Can One Spend
At The Center?
There is a choice of day or evening programs, Monday through Friday
plus an optional Saturday program.

Door to door ambulette transportation
Full medical and nursing daytime care
Delicious, hot, fresh meals in a congenial atmosphere
Bi-lingual staff: English and Spanish
Stimulating recreational activities and the opportunity to
socialize with peers
Medical and nursing supervision and monitoring
Individual needs’ assessment to help improve and maintain
independence
Nurses who observe and monitor health status, administer and
monitor medications and perform catheterization, dressing changes,
etc.
Physical and Occupational Therapists
Speech Therapists and Language Pathologists
Nutritional services including dietary evaluation and instruction,
with special attention to dietary restriction
Psychological services
Personal care
Appropriate referrals to community resources
Social service professionals who offer family and participant
support
Social and recreational activities
